Case Management Services
After determination of eligibility, the case manager will meet with the customer and do an in-depth assessment. At this time barriers to employment are identified and an appropriate training and/or employment plan are discussed.
An individual service plan is completed to identify realistic and attainable goals. Linkages to training agencies and employment services occur.
After private sector employment is secured, there is a 6-month follow-up period, which assists with job retention and upgrading.
